I asked the Administrators about that also, but never got an answer.

Although, they did approve an appeal I made (shortly after) for removing two post that I made reviewing an author's book series, by comparing it to my book.

that I don't know. I think they are sort of placed automatically by google or something based on content of the pages they are on.
