He does not understand why he is popular among Republicans, yet not popular with Democrats. He struggles through a few sentences and then explains that he is in favor of war with Iran and a supporter of Israel's actions in Gaza. Thus, he underlines his confusion.

I submit that Fetterman's confusion is the flip side to Susan Collins' frequently bouts of "concerns"
