9. IDK, but I'm guessing by default at DC office, but applicants can say "No Thanks. I'd like the standard format."

And of course, you can avoid the whole thing, even if DC is your closest office, as long as circumstances don't require the application to be made in person (e.g., no need to be expedited for some reason, which I don't think is possible online/by mail.)
