I don't think this is new. It's just been noticed and reported on now.
I bet it has been noted in physicals in past years, but he always covers up his physicals.

This is what Nixon would call a
'limited hang out" when he was in the white house.

They still aren't telling us the complete stories of his physicals, nor the medications he takes, such as whatever infusion they are giving him in the back of his hand.

I am guessing he has an IBS condition and they are giving him Remicade infusions.
