but he looks like an absolute freakshow, every day, to the rest of the world (I mean physically, his behavior is another albeit parallel matter of similar description).

The fact that he goes out in public looking like that is an indicator that nobody around him cares enough about him as a person to stop him from looking like that -- an appearance by any metric, even his own unreasonable ones, which ought to be embarrassing. Granted, it would be an enormous and painful and thankless task to undertake. But still.
